Non receipt of ATM card

I am holding a saving account NO. 0221101000535 with Canara Bank, Chunabhatti Branch Mumbai. I have applied for ATM card in November the form was lost by the bank. Again i applied in the first week of March however till date i have not recived any communication from the bank, whereas my mother who had applied along with me in march has received the card.
