ATM card Blocked

Sir,

I have an ATM card No. is 4214 5828 8200 1481 which is valid from 06/08 to 06/13 by the Name of Rishi Jain which is issued from Sonepat, Hindu College Branch.

My card was blocked yesterday by mistake.

Can i know the procedure for unblocking my ATM card so that i can freely use that card again.

My password was either 2143 or 1486.

Please provide me the solution soon.
