Subject:- Request to rectify/modify the details in the Policy No.4000061897 i.e. a Child Care Plan.
This is to inform that I have invested Rs.1- for a Child Care Plan in the name of my son i.e. Shri SANAT MANAS for an annual premium and its policy term 10 years. After depositing the money, I have received the deposit slip wherein my policy number is 4000061897 and client ID No. is 100132431.
In this regard, it is to inform that there are a number of changes in the policy that is required to be rectified/modified and the same is mentioned below -
01. That the name of my son is published in the policy as Master Sun Kumar whereas I have clearly mentioned the name of my son in the IDBI Proforma as SANAT MANAS.(copy of birth certificate enclosed)
02. That in the prescribed proforma I have applied the above policy for 10 years whereas the same is mentioned as 20 years.
I am unable to understand how this poor mistakes are happening in your reputed insurance company, therefore both the above details are required to be rectified/modified so that in future, I may be able to continue my policy. And on maturity, no hindrance/restriction may be occurred to receive the matured amount.
You are requested to kindly instruct your concerned officer to change /rectify both the above details in your record and issue me a revised deposit slip/document so that I feel secure myself from this difficulty.
