Imposing of Policy

Citibank has given me a Mediclaim policy in month of Aug on behalf of my credit card account with sending any prior notification/documents. I was surprised when I got policy from citibank and they have deducted this policy amount from my account. When I call back citibank regarding this policy cancellation they said that we can refund only 50% because of your cancellation period is over for 100%. But I told them that you have not given any notification or reminder that we are going to issue a policy of 7900/- .

Citibank has not given me 100% refund on this imposed policy and they are asking me to pay rest of 50% amount which I have not used. I have given a letter to Citibank manager to close my account with some amount after consulting with their executives. But citibank is denying this also that they have not received any letter and their executive is also not in there bank.

Please suggest me how to tackle this issue because I really frustrated and mentally disturbed about this.
