Cooperative Fair Price Shop - Non supply of commodities Irregularities

by the sales man, Mr. Palanichamy - Action Requested.

I am residing at Door No. Old No. 50/ New No.80, Ayyampatti North Street, Srivilliputhur in

Virudhunagar District. I am having family Card No.26G-0269396 which is attached to Mangapuram-2

Co-operative Fair Price Shop. The Salesman, Mr. Palanichamy is doing following irregularities with

connivance of unauthorized person, Mr. Selvaraj in the Fair Price Shop and spoiling the image of

Government.

1. Non supply of essential commodity to my Genuine Family Card No. 26G-0269396 (A NoA07).

Without actual supply of commodities to real card holder, the Salesman, Mr. Palanichamy is

selling commodities to smugglers in Block Market by Bogus Billing for several months.

2. Family Card Holder, Mr. Balu Naidu (Wife Name MrsJanaki) - Both expired 2 years ago Card

A No.361 - Still, the erring salesman, Mr. Palanichamy is doing sales to such Bogus card

by Bogus billing.

Due to high irregularities of this salesman, there is a heavy loss of Government money in Public

Distribution System. This salesman is also spoiling the image of Government. I therefore humbly

request you to take immediate action against erring salesman, Mr. Palanichamy and inform the fact to

me.
