Pf not released after 1 yea

My pf no is KN/35224/013584 and my epm. i.d. is 13664.I have worked for teamlease company from nov.2004 to apr.2006.I have submitted the form of pf withdrawal and they releasedmy pf cheque no.93053&978795 but due to name mismatch in my account i returned the cheque and send again pf re -application form to the epf banglore office on 15/07 through blue dart currior pod no.is 12076972692 but till date i am waiting for the same. I have tried na lot of time to contact Banglore office but there is no satisfactory response from their side plz help me on this.thanks,
