Opening for Demat A/c

On 29 sep2008 i have applied for opening for ICICI Demat A/c,

i am working in ESSAR Group.

till today I have unable to know my account status,

your sales manage Manish Tiwari from Borivoli Branch, he told me within 7days should be opened your Demat A/c.

but till date i don't konw about any staus from your side.

My old form : 9403341145 rejections due to

(

1. NOMINEE''S DATE OF BIRTH REQUIRED ON FORM.

2. SCHEME PROOF REQUIRED.

3. BANK STATEMENT SUBMITTED SHOULD BE LATEST OF THREE MONTHS.

Our Sales executive from BORIVALI - REGION will get in touch with you for rectification of the ")

whenever i called your sales maneger,

who didnot pick the phone.

can you let me know please my Demat A/c Open in you Bank other wise i will go

other Demat service provider bank.
