Loss of PAN card

To whomsoever it may concern,

I, a resident of Mumbai, have lost my PAN card on 13 February. The problem is that I do not even remember the number all I know is the name on which the card was registered. Please help me as to what can be done about that. I also want a duplicate PAN card please tell me the procedure.
