Account opening kit not received

Dear All

I have applied for a new NRE account in HDFC Bank. I have called several times at HDFC Dubai office regarding this, but didn’t get justifiable answer. My account no is 13251060000057. I asked bank to deliver "Account opening kit" at my Dubai address, but they sent it to surat. And now they are expecting me to fill a form for re-issuing the kit. For that they might charge me.

It has been more than 1.5 months when i gave all required documents and demand draft to the bank for opening an account. Please help me in this regard.
