Re issue supply

Respected Sir/ Mam

I have a gas connection in Avad Raj Agencies, Mulchand Path Patna. My consumer no-606811.Iam out of Patna for the last

8 yrs, so that i have never purchased the gas from the agencies.

Now i am returned in Patna. Now i want to re issue the supply.

My address also has been changed, please conformed me tha what is the procidure for re issue the supply.
