Pending PF withdrawal Request

Dear Sir/Madam,

I, Sudhir Kumar Shah, an Ex-employee of IBM DAKSH Business Process Services Pvt. Ltd, Millenium Tower II, Sector V, Salt Lake, Kolkata-91, Employee ID - 41996, PF a/c # WB/CA/45000/0255, have personally submitted the PF withdrawal form by duly filling it up with Kolkata, Millenium Centre Office in the month of April. However, even after several follow ups, no body has bothered to take any step towards fulfillment of the same.

Therefore I request you to take up the matter with IBM Daksh, Kolkata, Millenium Centre Office and kindly reply with the status of the same.
