I have deposited the premium of Rs.40, 000 in favour of ICICI Prudential Life Insurance by cheque No.009722 431016002 dated 04/03 on Central Bank of India against policy Nos. 02529228 Dr. Soujanya Machiraju for Rs.20, 000, Pol No.00852801 Dr. Venkatarao Machiraju for Rs.10, 000 and Pol No.00857657 Dr. Soujanya Machiraju for Rs.10, 000. The cheque was acknowledged by your Jalna Branch on 04/03. So far I have not received any receipt acknowledging the premium paid. I have to file the income tax returns in which I have to claim deduction. Your office at Jalna is now closed. I would request you to immediately mail the receipts on my email address in order to make the claim in income tax. The last date for filing the return is 31st July. Please help me.
