Difference in name in documents and in riffiling reciept

i have got indane gas connection in 2006 my name is A.L.Sharma documents show the right name but in reiffiling reciept the name is shown as L.P..sharma I requested many times but a gency didn't correct it. PZ look in to the matter seriously &give them instruction to correct my name.
