Non receipt of Monthly season ticket

Dear Sir,

Today i have booked the season ticket at Centraly Railyway Kopar Station (Thane, Maharashtra State) from Virar to Dombivli via Dadar Rs295/- through my SEASON ID Card No. B0QL9PM0B7 IC NO.3680 the Clerk Mr. Prashant Bhalerao has taken the printout of the same & ask me to give Rs.5/- as change meanwhile the printout of the season ticket lost or some other passenger has taken the ticket. when i given the Rs.5/- to Mr. Bhalerao i asked for the season ticket but he told that the season ticket is already issued. So i told him that i have not received the ticket. but he inform me that he is enable to re issue the ticket. He has given the UTS NO.G56L9XB4B7 & I 95417643 for reference.

Requested to kindly note that i have already paid the season ticket charges but i have not received the season ticket. I am regular customer of Indian Railway from last 18 years.

Kindly suggest what to this in this case. if railway are enable to re issue the ticket then i have to bear the lost of Rs.295/-
