Non-receipt of Annuity

I wish to inform you that I have not received my annuity since 1st March.

My annuity is normally credited to my Bank account with HDFC Bank, Pali Hill Branch, Savings Account no 00161000082421

When I did not receive my March Annuity, I assumed that I was required to submit a Certificate of Existence, which I did on 21st March at your Bandra branch. This certificate was then sent by them to your Bhandup West branch.

Please restore the payment of my annuity wef 1.3.2012.
