Looking for clearance for lost passport

Dear Sir / Madam,

RE: Lost Passport Ticket # 177374

In regard to the above ticket number issued by your embassy in reference to a lost passport. I applied for a replacement passport on June 12th in person at the embassy in Washington. I was told by the embassy personnel to pick up the replacement passport on July 12th between 1630 and 1730 hours. I appeared in person at the stated date and time to pick up my passport and was informed that they were unable to issue me a passport because my application was missing a passport number on the original application I filed for renewal of the passport. I gave the embassy personnel my old passport number and they informed me I should hear back soon. As of the date of this letter I have not received any communications from the embassy.

I would appreciate it if I could be contacted regarding the status of the application for replacement of a lost passport. My old passport number is A6489415 and was expired on December 11th.
