Dear Sir/Mam,
I frquently lounches the complaint regarding your statement.
I didn't recieved your credit card.
when the card is deliver from you that time i refuse the card delivery because of the name on the card is not correct.
So i am sent the same card to you.
After that time to time I am complating the same thing.
But no response from your end.
I didn't have your card but through sms your statements are regularly received.
So please confirm me the status of the same.
