With reference to your mail dated:21/01/09 TTK CRM #108129 with reference to your email I undersigned hereby inform you that I had sent email on friday 19/12 against your reply for your email dated 12/12 for the card number. Hence no specific card no is given to us. National Insurance Compant Ltd started Floater Group Medical Policy to the members of our bank under G.M.P No:260800/46/07/8500000055 which is common to all the members being a group policy. We provided documents enclosed in support of your claim:1.Original Discharge Card.2.Original Test Report prescribed by Doctor.3.Original Bills, Receipts and cash memo etc.4.Prescription for clinical report, medine etc.5.From Hospital and/or Surgeon/Consulting Doctors Prescription.6.Date of admission 31/07 and Date opf discharge 10/08, zerox statement sheet of Vyas Surgery Hospital Sion Mumbai .7.Zerox copy of Intake and output chart. I was hospitalised from dated 31/07 till 10/07 in doctor vyas surgery for the treatment of slip disc werein I had occured total expences of Rs.38000 which was previously understood by me that the same will be reimbursed to me after hospitalisation on submission of the relevant document as per company's process. I received a letter from Company that above hospital is having only 8 beds and that the hospital should have atleast 15 beds. We already paid premium Rs.3000 against medical allowance and the same amt is being provided by the bank on yearly basis for its members. But this fact was naver known to me and in case if any emergency occurs is it quite legitimate to find such hospital which will fulfill your norms or we shall have the right treatment as per the need of the hour and the same was done by me the case being critical. With refference to mail received from you I would like to say that being group medical insurance policy (Floatal Policy) was given to me from National Assurance company Ltd, card was not given to me being a group insurance
It learnt that previously some members recived the cards, after they stoped giving cards to all of the members hence I am not having cards as I could not recived the same. The company started group insurance after stopping the cards.
Please look into the matter and solve the promblem sympathetically,
