Lost my Pan Card in the month of May - 2008

Hi Respectables,

I am Komala devi working as an employee in Numbers Only Inc, INDIA - Hyderabad.

I Lost my PAN CARD in the month of May - 2008 in Hyderabad city.

I have re-applied to get a new card with the same number - AJUPD9741L CSF number - 702617952 but returned due to miss match in the name about space between names. I called up to UTI services in hyderabad, but still didnot get my card.

Can you please reply me for further proceedings, what should I do to get a new card.
