Wrong name on PAN card

Hi,

Iam holding a PAN card since 2002.The name appearing on my PAN card is incorrect.

Name: SHRADHA.PRABHAKAR.BODHADE.

Name appearing on PAN card: SHARDHA PRABHAKAR BODHARE

Earlier, I had given request for change in name but has not been done till now.

My PAN number is AGJPB8763C.

Please help me in getting the correct name on PAN card. Recently I have also received income tax refund cheque in wrong name. Please help!!!
