Not returnning the original document since last 4 month

Dear sir,

I would like to inform you that your one of the staff name-Kamalkanta basai (Raipur) has taken my original documet since last four month and still not yet return to me. If i am trying to contact him then he is not pick-up the phone. Therefore please it is my kind request that help me out from this problem. Finally i bound to write to you.
