Pan card not received

Hi Team

Still i am not get my pan card. First i lost my pan card, then i applied for another new card for that. On july last week i got letter from UTI department, It regards name mismatch between application and old pan card name. I sent reply on 3rd august itself what are they require for further action.kindly advice me status. My pan details are following

PAN no: AGLPT2391N

COUPON NO:34769268
