Not recieved pan card

Sir,

i have applied for the pan card before 2 or 3 yrs . but not get the same till date, thuogh i have recieved pan no.-ACQPG4916P i have made several effort BY my incom tax lawer but nothing happened.

as i have to file the return plz do issue my pan card on following address:
