PAN CARD not received

I Heeramani sahu applied for PAN NO. & I recieved my PAN CARD NO. BVBPS1582C

ON April. but same was lost from me due to pocket was lost

again I applied for 2nd PAN card dully filled form which i collect from wealth creators NSDL

centre Bilaspur on NOV. 08 but till i have not received my 2nd pan card

Again I have Problem that is I also lost my acknowledgment so Kindly help me
