Dear Sir,

I, Mrs. Daisy Baishya have applied for a PAN Card in 13th Octubar at UTI Guwahati. But i did not get it till today. My Processing fee coupon number is 029743036 . My application SR. NO. was 042216043. And at that time a PAN Card number was given to me against my application in written in the form. The PAN Card Number is ARNPB2558A.

Sir, So far i have not received the PAN Card. I approached the above said office. They said it was sent to my address.

We request you kindly to look in to my complaint and arrange to send the PAN card. I will be greatful to you if you give a look into the matter and do the need full.
