Minor Passport not issued - URGENT!!

My kid's passport is not getting issued because birth certificate has my name with middle name.

Had applied thru tatkal, its been months but no response. Police verification also done and in site it is showing as delivered when enquired it seems it got rejected.

Please need help it is very urgent...
