To withdraw PF but comp. shifted

Dear sir,

As i was the employee of pfimex pharmaceutical (hyderabad based comp.) wanted to withdraw my provident fund for that i send duly filled & signed PF withdrawal formbut unfortunately comp. address shifted to somewere else which i dont know so i am getting difficult to withdraw the same.

Please guide me.
