LR no. 1722 dated 26/04-Ex-Hyderabad to Trivandrum.
This is to inform you that under the above lorry receipt, household articles containing 97 items were given to you for onward dispatch to Trivandrum. At the time of loading the consignment in your vehicle, we were assured that the same vehicle will carry the goods to Trivandrum and the delivery will be made within 3-4 days. However the goods were delivered on 7th May after 10 Days with multiple calls from our end.
You have charged a sum of Rs 3840/- towards insurance charges and Rs 3332/- as service tax.
It was assured that the Insurance document will be sent in 24-48 hours. We have given our email address for the same. We are very sorry to inform you that so far this was not dispatched to us.
The above consignment was delivered to us fully damaged/pilferaged condition by another lorry. Due to repeated loading and unloading, most of the items got damaged and also we noticed that some of the cases were forcefully opened and repacked at your end which are still kept aside without opening and the picture has been taken in front of the local police for future references.
Since the same day, I was contacting you over telephone requesting to send us the relative insurance certificate to enable us to claim with the concerned insurance company. Though you assured to send it immediately, till date the same was not received by me. In the absence of which, I am not in a position to lodge the claim with the insurance company.
Also please note that it is clearly mentioned in your invoice that the goods and personal belongings not meant for any kind of sale and are exempted from all taxes and control wide authority Act No.2, Section 1580 of the Indian tolls Act 1901, whereas you have charged Rs 3332/- from us as service tax which also should be paid back immediately.
We request you to issue the Insurance document as the cost of damage is high and
