Not received pan card

Dear Sir,

My uncle AJAY KUMAR JAIN (DAMOH -M.P.) has not received his PAN CARD.

He applied on 26/08/08, application no.038824895, coupon no. 27773735,

PAN no. alloted BDIPK9688J and PAN CARD despatched on 15/09/08, BUT,

he has not received his PAN CARD yet.

Sir, please tell me what to do next, in order to receive PAN CARD.
