Not issueing passport

R/Sir/Madam

I would like to inform you that i have applied for my Passport seven month ago police enquiry has been cleared my file No. BPL-A026941 My Name is Garima Jain. Earlier the status was "paasport will despatch on 11th march", but now status is showing "Please furnish 3 Personal particulars forms, duly completed for verification", please tell me what should i do?

Therefore i request to you kindly issue passport as your earliest posible for this i shall higly oblidge.
