I Mrs. Jamuna Shetty who took birth on 05/09/1949 at Nair Municipality hospital, Mumbai; till date I haven't got my birth registry certificate from the Ward Office of the respective Medical Officer of Health. They are infact denying my birth at the hospital for reasons better known to them. Please furnish me with a copy of my Birth Registry Certificate.
