Incorrect Middlename

Hi,

My name in pasport is given as on front page as

Mehta Kunal Pravinchadra

And on last page it is given as

Mehta Kunal Pravinchndra

Correct name is

Mehta Kunal Pravinchandra

Notice that alphabet is missing in front and last page is different. can someone please let me knoe what can be done. Can affidavit suffice.
