Passport not recevie

I had Applied passport Application Date : 01-05 both police and CID verification has been done, but i check the status on web still showing Police has recommended for Police Verification from your permanent address. Please furnish 3 personal particulars forms of permanent address.but i m alredy furnish 3 personal particular forms, and again police and CID verification has been done.but i have not received my passport. File Reference Number: GZB-z641031 And dob 20-10-1985.
