One year delay-not available passport

Espected sir,

i am shaik ahmed, one year before i had submit my passport for renewal- file no: D003930, but still i didnt receive my passport. three times they asked me to submit PP form i submited that even than no action., because of passport delay i lost lot of abroad chances therefore i approch consumer complaint i hope there will be good solution.
