TELEPHONE NOT WORKING, AND BROADBAND CONNECTION BREAKAGE

My telephone connection have certain problems, the phone is dead and broadband connection keeps breaking.

I registered a complaint with the customer care and local telephone exchange but they are not turning up to assist me,
