Disconnection of Telephone and Broadband

I have telehphone and broad-band connection. My telephone number is 0129-2512407. Around 10 days back, I have given request to disconnect my telephone and broad-band. I have submitted the form after taking NO dues. I also submitted the telephone equipment. Disconnection is yet to happen.

I request you to kindly disconnect my telephone and broad-band.
