Regarding to the DD clerance and not given Band

I Aravind C from Davanagere. And I applied the proposal to Birla Sun Life Insuance on 8th October and DD No 315772 was taken on the same Date with the Davanagere Co-Operative Bank. and the DD was collected on 22/03, till now I did not get any receipt, policy number or Band regarding with this, So please take action on this and give me the truth.
