Delay of issuing debit card

My account services all are freezed due to delay of submission of updated documents. after 10 days, i had submitted all documents. after 15 days, submission of documents they are not activated my account and my debit card also. one day i asked in my account branch, but they did not reply anything, iam fully fedup and frustrated with this bank.
