Payment without notification

I Mrs. Santha K.Nair holder of policy number 11235886 wishes to put forward the following:

1. I have taken this policy on the 23rd of august for a term of ten years as per the policy document expiring on the 22nd august. I had taken this from appus towers, 1st floor, near new bus stand, payyanur 670307.

2. I have received a cheque from HDFC vide chq number:04779366 dated 31/08 for Rs.4, 99, 380.01 under client ref no: 11083140055353.

I would like to know the following:

1. why this cheque is being sent in 2011 when the expiry was in 2017 without even giving any explanation and without my knowledge.

2. When I have paid 500000 as the premium in 2007 in unit linked programme why without my consent the same has been liqudated at lesser price than what I have paid? For four years of giving money of 500000/- I have received payment of 4, 99, 380 which is less than what I have invested.

I need specific explanation to the above matter ASAP. I have given my birth date for verification as under
