Icici prudencial life insurance
Debited to a/c without confirmation

Dear sir,

This is to complaint that the amount of Rs-8579/-has debited to my hdfc a/c no-04801200000334 to without my confirmation/knowledge. When i claimed the hdfc bank for wrong transiction then the bank has provide me a complaint no(#MUM253389) and call to me after two days that the amount has deducted to your a/c through netbanking with icici prudential life insurance towards transiction no-006003500600530870317420110329.For information i shall want to say that i do not use any kind of netbanking & no any facility are available on my general ATM card. After this transiction my hdfc cheque has bounced of the amount of Rs- 15000/-.So, i claim the 5*time amount penalty of bounced cheque amount.

Therefor, i request to you that please do the instuction of hdfc bank & following insurance company.because i am a private company job worker so, i am unable for run up to following branches.
