Pan card not recd

Dear sir/madam

I have applied for relacement of PAN CARD because of my existing pan card is damaged to Income Tax PAN Services Unit, National Securities Depository Limited, 3rd floor, Sapphire Chambers, Near Baner Telephone Exchange, Baner Pune -411045 on 12.10.2010 with prescribed fees of Rs.94/- and alongwith all the relevant documents such Old PAN CARD, electricity bill, telephone bill, voter id card, copy of bank pass book, letter from employer, matriculation certificate etc. but till date they found several discrepencies in my proof of identity and mismatch in the last name i am attaching their email response with this:-

This is with reference to your PAN Application made vide

Acknowledgement no. 881020203439272. In this context, please note that we

have observed following discrepancy (ies) in your application.

1) Name in Proof of Identity does not exactly match with last name as mentioned in application.

I am submitting all the documents i have and no more documents i have to submitted .as this not a fresh/new application but it is merely a replacement of old PAN CARD with The NEW PAN CARD with the same old PAN NUMBER
