Sub.: Non receipt of cheques-ECS Mandate-Reminders

Ref: Annuity No.017104

I have for reference to my earlier letter No.: HS/LIC/06/-07/26 dated 9/12, 12/04/07, 20/10/07, 20/12, 05/02/09 & 10/04 in regard to non receipt of annuity cheques for the period of July to Jan. I have received annuity cheques for the period of April on wards received by me. Subsequently now it is being directly deposited in Bank of Baroda through ECS.

I once again give below details for your ready reference.

Please take a note of above details and arrange to send my above non-receipted cheques of annuity on urgent basis. In other case directly deposit in my above bank account through ECS under intimation to me at the earliest.
