Wrong policy issued which was not asked

Sub: Request for cancellation of Policy Document No. 20230461 issued in favour of Sh. Satyendra Sharma.

Sir,

I, Satyendra Sharma working as Addl. Executive Engineer in the Rajasthan Vidyut Prasaran Nigam Limited, had applied for SIP Plan of MetLife mutual fund in the month of January through Ms Neha Mathur from Bajaj Capital limited, Jaipur. She visited my office as well as my house and got signed by me a one-page form at place only for the SIP plan along with a cheque of Rs. 6000.00. At the end of the III week of January I received the Policy Document which was lying in shelf of the staircase of my residence. I was shocked when I opened this policy document. It was altogether different document from the one, which I had applied for. The details of the policy document, which I received is as given below;

I want to share some astonishing observations of this policy document. These are:

1. I had never filled and signed the policy form that was enclosed in the policy document. I had signed a single page only at one place.

2. Instead, there were seven signatures in the form attached with the policy document, which surprisingly I had not signed. Besides it can be easily noticed by any one that all the signatures differ from each other.

3. The information about the good health of my parents is incorrect. In fact I am sorry to say that they are no more in this world. They passed away much before (mother in the year 2000 and father in 2005).

4. The information about my other family members in the document is also wrong.

After receiving the above policy document I contacted Ms. Neha on mobile and inquired about the wrong policy document and the information therein. Ms Neha explained that it was done inadvertently and assured me that the same will be rectified in due course of time. Ms Neha collected the original policy document in the next two-three days. I also signed the application for cancellation of aforesaid policy document sent by her.

After above formalities I thought that the cancellation would soon be done by Metlife in due course of time. But, I was surprised to receive a notice from MetLife for depositing the next installment, which fell on 11/March. When I inquired from Ms Neha about this, she advised me that I should withdraw all money from my Bank so that the ECS due on 11th March could not be materialized. I did as she said with the hope that the policy would be terminated in near future. I have being doing this (withdrawing money from my savings bank account) month after month till to date. So far Ms Neha is continuing with her fake assurances that the necessary action will be soon taken.

Soon I received a call from MetLife, Gurgoan regarding non-payment of my monthly installments. I explained on telephone to the caller about the happenings who told me that they will take some action to resolve my grievance and also advised me to send a mail on [email protected] regarding my compliant. Meanwhile Ms Neha contacted me and requested me to write a letter addressed to Bajaj Capital, informing that I had no specific compliant against her but only wanted to terminate my policy. I conformed to her request in good faith. Unfortunately Ms Neha has done nothing to free me from this policy document, which I never wanted.

I had contacted many times to Metlife on their toll free number and also sent e-mail as well as through fax and registered post regarding cancellation of this policy document but no action at all was taken by the Metlife. In fact I received not a single call from Metlife.

Today, I am writing this letter to you as I am fed up from all this inconvenience and tension that I have faced in the past ten months. It is, therefore, requested to arrange to cancel the policy document number 20230461 issued in the name of Mr. SATYANDRA SHARMA at the earliest and make arrangement to stop ECS from my savings account number SB51090681903.You are also requested to make arrangement to refund the amount which I had deposited at the beginning.

I firmly believe that necessary action must be taken against persons like Ms Neha Mathur and Metlife representatives so that they may not indulge in such illegal activities (like false signatures and information) in future.

An early action is solicited.
