Kindly allow me to lay down the following few lines for your kind attention and necessary action at the earliest.

1. I am senior citizen bearing LIC Policy of Rs 2, 66, 665/ - Varishta Pension Bima Yogana bearing Policy No : 835059769 . I had received 12 cheques of Rs - bearing the amount of Pension installment in advance from your office.

2. I regret to bring to your kind notice that due to oversight, I could not present with in 3 months the aforesaid cheques bearing no 028334, 028335, 028336 drawn on Axis Bank. As such the same has been returned to me by the HDFC bank, Darpan Branch, Ahmedabad. All three cheques had been sent to you via speed post to above address on 11/03.

3. I further request you to send me the Form for ECS payment per return of Post in order that I can avail the facility.

4. I am senior citizen running 74th Year, hence request for an early action per return of post preferably by speed post at the above address.

We have already sent all three cheques to your ahmedabad office address mentioned above on 11/03 by speed post. A copy of these letter is also sent to The Grievance Redressal Officer, SR/BR Manager, CBO 13/857 / . Dr Kastubhai Chember, Nr Gandhi Ashram.

I request you to pl look into these matter and do the need ful at the earliest.
