Sir,

I am an ex-servicemen. I had taken Loan aginst my Policy No APS - 447027-L in the year 08. My Original Cerifiate for the above policy was deposited in APO Mamun Cantt.

My loan was cleared by Receipt Book Bearing Account no. TD: 15963 in the year 2008 August and the book was deposited at your Sariani Nagar (Netaji nagar) Post Office with the representatve who deals with Signals.

I am sorry to inform you that after several folllowups till date I have not received my Original Policy Certificate.

You are requested to look into the matter personally and do the needfull.
