Wrong PAN card number issued

I was allocated a PAN number AFXPA4112M by UTI bank Belapur branch in 2004.

I have been paying taxes using the same PAN number for past 6 years.

Today when i tried to open an account in some bank, then it was brought to my notice that the PAN card does not belong to me, but it belongs to some MR Mahadev.

Can some one please look into this discrepancy?
