Cheque return due to bank mistake

Dear Sir,

I have a saving account with PNB since Oct'05. My name is Mithilesh Kumar Sinha. I added My wife Reshma Sinha as joint account holder with this account in 2007. My account has been transfered from Sarvodya Enclave, New Delhi to Dwarka Sector 10 on July. I have a good experience with PNB but at the first chance there is an unique problem occured today. I received a call from PNB, Bhikajikama Place Branch (011-26169022) regarding my cheque no. 954246 of Allahabad Bank, Lajpat Nagar Branch for Rs. Ten Thousand which is going to return due to the mismatching of Name in Cheque "Mithilesh Kumar Sinha" and in account only "Mithilesh". When I recall the same no. and request that my name is Mithilesh Kumar Sinha not only Mithilesh you may check my detail wef 2005 but the person was not ready to listen me & disconnect the call. Then I call customer care who advise me to disconect the call and recall 6 button. I tryed so many times but the no is still busy therefore I am sending this mail to you. I want punishment of the responsible person who convert my name from Mithilesh Kumar Sinha to only Mithilesh. My a/c no is 0636000102166025.
