Funds swaped without consent, policy document cancelled in Jan

Policy holder got cancelled the policy document and it was duly cancelled by the company, no policy document was sent by company.however the policy was made, even then the document were not sent. After confermation it was found that the money has been swaped in form of new pol.docs.without the concent of policy holder. Hence it is requested the pol.doc money be refunded.
