I am furnishing the detail of my policy for your early action:

Date: 2.7.2001

Name: Triloki Nath Datta

Purchase Price : 50000/-

AMOUNT OF ANNUITY: 2610/- HALF YEARLY

Dt. of Ist annuity: 1.1.2002

My half yearly annuity was due on 1st Jan. 2010 for Rs.2610/- which I have not received. I have neither received the cheque or through ECS. Bur since 1.1.2002 onward I have been regularly receiving the payment through the cheques now except for 1.1.2010. Some months ago I was asked the fill the form which I submitted to your office along with ECS Form and live certificate. But the payemnt

in question has not been sent to me through ECS and no cheque in this regard is received by me so far. Please look into the matter.

For your information again I am providing you the detail of my SB A/C also.

Please send the payment through the cheques or through ECS:
