Dear Sir/Madam,
I am holding total 100 unit (One Hundred Only) shares of 50 each of face value of Rs.10/= each under Folio no: S06990 and Certificate no 159445 & 159446.
And I wish to bring to your kind notice following facts in this regards:
That the shares were secured in my name by my father Mr. Ajai Mishra in Year 1994, that time i was minor and we were residing in Dhanbad (Jharkhand) and now shifted to Varanasi (U.P) in year 2005 at above address.
I understand that the face value of above shares has been changed from Rs. 10/= to Rs.5/= as such I am supposed to get new shares, which I am yet to receive.
Under circumstances explained above you are requested to transfer 100 shares under folio no: S06990 & certificate no: 159445 & 159446 in my DPID no: 1203320005162189, cancelling the shares issued in physical form.
